Eligibility
Personal Health
The ISD Employee Health Clinic is available to the following individuals enrolled in the Independence School District health insurance plan:
Employees
Pre-Medicare retirees
Spouses and dependents age two and older
Medical COBRA participants
Work Related
The Clinic will also provide first treatment of work-related injuries to all District employees regardless of health plan enrollment.

[3] Due to IRS regulations, HSA eligible plan members must pay a minimal visit fee for non-preventive visits, including chronic care or other significant benefits. All fees apply towards deductible and out of pocket maximum. The visit fee will be re-evaluated on an annual basis.
In order to maintain the tax benefits afforded to HSA participants, the IRS requires a fee be charged to those with a Health Savings Account (HSA) for non-preventive visits. To be able to contribute to an HSA, the employee must be covered by a Qualified High Deductible Health Plan. Many employees setup their HSA through ISD; however, some have an HSA through a spouse’s family plan or may have previously setup a HSA on their own or do so at a future date. This makes it very difficult to identify all employees on the Qualified High Deductible Health Plan that may have an HSA. So, in order to meet due diligence under the law, ISD requires all Qualified High Deductible Health Plan members using ISD Employee Health Clinic to pay a per visit fee for non-preventive services. This fee is still considerably less than you would pay for similar services at a physician office, urgent care center, or retail clinic (such as Minute Clinic). Keep in mind that preventive care is covered 100% by the plan and will not have a fee. Once your deductible is met, all visits will be at no charge.
